Yeah all 3DMG is adjusted and fitted to each soldier, not sure why that one guys 3DMG had a trigger that was tough to pull. Perhaps it was his prefference? I'm not 100% on what could be altered about the 3DMG to make it adjustable though, really all i could assume are the belts but i don't see how that would effect anything.

For the one Jean picked up, it's probably a combination of both, or maybe size or wear down or whatever.

The body that Jean grabbed the 3DMG gear from was just that - a body. The reason we don't have scavenging in the game is 'cause the general consensus is that most people are pretty badly beaten up when killed by Titans (chomped, crushed, smashed, that sort of thing), thus the gear will probably be in an unusable state. It only stands to reason that there was a good chance that Jean grabbed damaged 3D gear. If anything, he was really lucky.

Game Suggestion
It would be nifty to have some sort of similar mechanic in Hard mode - if you hit the ground too hard, your 3DMG becomes damage, or broken if you hit it really hard. You can then a.) be carried back to the supply depot by another player,
b.) Attempt to scavenge 3D gear from a dead player. There is a good chance the scavenge will fail (hooks just won't fire after changing), a moderate change the gear will be damaged (hook(s) delay a second or two after firing), and a minor chance it will work fine.
c.) Make a mad dash for the supply depot.

This would of course work better with the dodge using up gas, so 3DMG will have a delayed dodge if damaged and you can't dodge if broken.

Having my 3dmg gear break all the time would just make the game annoying as fuck rather than fun and challenging.

what ever happened to blizz lol, haven't seen him around. Also I'm pretty sure that 3dmg is mass produced, but can be adjustable for personal preferences.
